Locate in-network doctors and facilities by selecting the tab for your network and following the instructions. No referral required to see a specialist.

Follow the instructions below to begin your search.

1. Click the button below - Doctors/Facilities Search.

2. Click the link - Basic search as a guest.

3. Using the drop-down menus, select the following:

  • Select the type of plan or network - Medical Plan or Network (may also include dental, vision, or pharmacy benefits)
  • Select the state where the plan or network is offered - Colorado
  • Select how you get health insurance - Medical (Individual & Families)
  • Select a plan or network - select Pathway Essentials HMO

4. Click Continue.

5. Enter zip code and begin search.

Anthem Mountain Enhanced Network

The following counties will have in-network providers: Archuleta, Eagle, LaPlata, Mesa, Moffat, Montezuma, Rio Blanco, Routt, and Summit.

  • How do I make a payment?

    Contact Anthem Member Services at (855) 453-7031 to submit your payment. 

  • I have a question about my plan benefits.

    We are here to help! Contact your broker or send an email to contact@wiswellinsurance.com.

  • When will I receive my Member ID card?

    Once your payment has been processed, Anthem will mail your member ID cards to you. Cards typically arrive 10-14 business days following the receipt of your payment. If you did not receive your cards, please contact Anthem Member Services at (855) 453-7031.

  • How do I contact Anthem Member Services?

    Anthem Member Services can be reached by calling (855) 453-7031. They are available - 

  • I can't remember the name of my plan.

    Send an email to contact@wiswellinsurance.com or contact Anthem Member Services to confirm your plan name.

  • Where can I access my 1095A?

    Login to your Connect for Health account. Scroll down to the section - Additional resources. Click the blue hyperlink under, "Do you want to upload or review documents?" Under the section - Past Documents and Letters - you will see a document titled "IND_IRS 1095A Tax Form".  Click the drop-down arrow, then click Download.
